By submitting my personal data I agree that my personal data, provided in the application form above, as well as my personal data related to my mental health obtained during the communication with me, will be processed for the purpose of the provision of Peer Support Program service to me (i.e. for assigning peer to my request, for contacting me, for providing me with peer support, for sharing my data with mental health professional - psychiatrist (in case of peer recommendation and based on program manager decision), in order for him/her to provide me with consultation and advice and, in cases when the threat to public safety is presumed, for disclosing my data to my organization (aviation academy where I study for becoming a pilot or work as a flight instructor), as well as to the airlines (in case when I am a student learning to become a pilot and I am a cadet in certain Airline Cadet program).

The consent may be withdrawn at any time by submitting a request by e-mail address [email protected]. The withdrawal of consent shall not affect the lawfulness of processing based on consent before its withdrawal.
